TRANSFEREES

  1. OFFICIAL TRANSCRIPT OF RECORDS (TOR)
  2. CERTIFICATE OF GOOD MORAL CHARACTER
  3. COPY OF GRADES AND HONORABLE DISMISSAL
  4. ORIGINAL PSA BIRTH CERTIFICATE AND (PHOTOCOPY)
  5. MEDICAL CERTIFICATE
  6. MARRIAGE CERTIFICATE FOR MARRIED STUDENTS (PHOTOCOPY)
  7. (3 PCS) LONG BROWN ENVELOPE WITH PLASTIC

ENROLLMENT PROCEDURE

  1. Account Creation and Registration
      • Wait in the SMMC Page regarding the opening of the Registration
      • Register. Fill-out every details. 
      • Wait for the confirmation about your day of examination
  2. Admission Test and Submission of other Requirements
    • Go to your respective Testing Room 30 minutes before the test.
  3. Notification of Results
    • Results will be sent to you by the Admission office to your registered email address.
